New Patient Resources & Information
Start Your Journey
When you arrive at our office for your first visit, we will need to see your insurance card(s) and your driver’s license (or other valid picture identification). We will scan these into our computer system so that you will only need to present them at your first visit of each year or if your insurance changes. Unless you have already completed the registration/check-in process through our Patient Portal, we will ask that you complete some forms prior to seeing the doctor.

What to Expect
A Podiatric Assistant will guide you to a treatment room, discuss your foot or ankle concerns, and review your medical history and lifestyle. You’ll need to remove shoes and socks from both feet. The doctor will perform an exam, run basic tests, and discuss your condition. If needed, x-rays or ultrasound can be done on-site with results available immediately.
- If you have medical records, x-rays or other diagnostic test results which may be relevant to the reason for your visit, please bring those with you as they may assist the doctor in his assessment, diagnosis or treatment recommendations.
- In many cases, treatment can begin on your first visit, but only after the doctor reviews and discusses the recommended plan with you.
- Because we do expect payment at the time of service, please see Your Financial Responsibility for an explanation of our financial policies
- Our physicians and staff members have only one purpose for being here – to provide you with the best podiatric medical care available. We appreciate your decision to “Put Your Feet in Our Hands”!
